Hourly Pay Rate: $56.95

What You’ll Do:
Provide psychosocial assessments and interventions for patients and families
Assist with discharge planning and coordination of community services
Support patients facing emotional, social, and financial challenges related to illness or injury
Facilitate referrals to appropriate community resources and facilities
Serve as a resource for palliative care and end-of-life decision support
Collaborate with healthcare teams to ensure timely, coordinated patient care
Document patient interactions and interventions in compliance with hospital policies
What You Need:
Required:
Valid LCSW license in Minnesota
Minimum 2 years hospital social work experience
Strong assessment, communication, and interpersonal skills
Ability to work collaboratively within multidisciplinary teams
Preferred:
Experience with discharge planning and community resource coordination
Knowledge of hospital protocols and electronic health records
