What You’ll Do:
Provide outpatient physical therapy services to adult patients
Assess patient needs and develop personalized treatment plans
Implement therapeutic interventions aimed at improving mobility and function
Document patient progress in compliance with facility policies
Collaborate with healthcare team members to coordinate care
Educate patients and families on treatment exercises and injury prevention
What You Need:
Valid Physical Therapist license in New Hampshire
Prior outpatient therapy experience preferred
Strong communication and patient care skills
Ability to work independently and as part of a team

This position is based at a critical access hospital in Colebrook, NH, providing outpatient therapy at a busy facility serving a diverse patient population.
