What You’ll Do
Assess patient needs and develop personalized treatment plans
Implement interventions to promote independence in activities of daily living
Collaborate with physicians and healthcare team members
Supervise OT assistants and students as needed
Complete timely documentation in electronic medical records
Promote a safe, clean environment of care using best practices
Uphold professional standards and ethical guidelines
Transfer and position patients safely during care sessions
What You Need
Valid Illinois Occupational Therapist license
1+ years of home health OT experience
Strong interpersonal and communication skills
Ability to work collaboratively within a healthcare team
Commitment to providing high-quality, evidence-based care
