Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Long Term Care in Claremont, NH

What You’ll Do
  • Monitor residents’ mental and physical well-being, promptly reporting changes to supervisors
  • Administer medications and treatments accurately in accordance with physician orders
  • Supervise and support CNA staff, ensuring high-quality resident care
  • Assist physicians during resident rounds and coordinate care plans
  • Perform wound care, oxygen therapy, and other treatments as needed
  • Document all care interventions thoroughly using electronic systems
  • Manage admissions, discharges, and transfers while maintaining compliance
  • Collaborate with residents’ families to ensure communication and address concerns
  • Maintain infection control standards and ensure a clean, hazard-free environment on the unit

What You Need
Required:
  • Valid LVN/LPN license in New Hampshire
  • At least 1 year of nursing experience in long-term care or similar settings
  • Strong clinical skills including medication administration and resident Observation
  • Ability to work independently and as part of a team

Preferred:
  • Experience supervising CNAs
  • Knowledge of infection control policies and wound care
  • Excellent communication and organizational skills
