What You’ll Do
Observe and monitor residents’ mental and physical well-being; notify supervisors of changes.
Accurately transcribe and follow physician’s orders regarding medications and treatments.
Administer medications, treatments, oxygen, and nourishments as per policies.
Supervise and assist nursing assistants, providing guidance and ensuring quality care.
Support residents during rounds with physicians and document care interactions.
Prepare residents for discharge or transfer, completing necessary documentation.
Collaborate with family members and healthcare team to coordinate ongoing care.
Maintain infection control, safety, and cleanliness on the unit.
Participate in resident care planning and ongoing staff education.
What You Need
Valid LVN or LPN license in New Hampshire
Prior long-term care (LTC) experience preferred
Ability to work independently and as part of a team
Strong communication, attention to detail, and compassionate patient care skills
Knowledge of electronic health record systems and medication administration protocols
