Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) – Long Term Care in Claremont, NH

What You’ll Do
  • Observe and monitor residents’ mental and physical well-being; notify supervisors of changes.
  • Accurately transcribe and follow physician’s orders regarding medications and treatments.
  • Administer medications, treatments, oxygen, and nourishments as per policies.
  • Supervise and assist nursing assistants, providing guidance and ensuring quality care.
  • Support residents during rounds with physicians and document care interactions.
  • Prepare residents for discharge or transfer, completing necessary documentation.
  • Collaborate with family members and healthcare team to coordinate ongoing care.
  • Maintain infection control, safety, and cleanliness on the unit.
  • Participate in resident care planning and ongoing staff education.
What You Need
  • Valid LVN or LPN license in New Hampshire
  • Prior long-term care (LTC) experience preferred
  • Ability to work independently and as part of a team
  • Strong communication, attention to detail, and compassionate patient care skills
  • Knowledge of electronic health record systems and medication administration protocols
