Hourly Rate: $50.25/hour

What You’ll Do
Provide skilled nursing care to patients in their homes
Document patient assessments, interventions, and progress accurately
Collaborate with healthcare teams to coordinate patient care
Implement care plans according to facility protocols
Educate patients and families on health management
Promote health and safety while maintaining independent judgment
Communicate effectively with team members and document all relevant clinical data
What You Need
Required:
Valid Massachusetts LVN or LPN license
CPR certification
Ability to work independently and make clinical judgments
Excellent interpersonal and communication skills
Willingness to work flexible hours and accept on-call responsibilities
Reliable transportation to travel within the service area
Preferred:
Prior experience in home health or community health
Knowledge of Medicaid/Medicare regulatory requirements
