We are seeking an experienced RN - Case Management to lead the planning and coordination efforts that ensure optimal outcomes for diverse patient populations. This pivotal role offers the chance to work within a supportive team environment, applying your expertise to improve patients’ transitions through the healthcare system and promote quality, cost-effective care.
Required Skills:
Valid RN license in the state of practice
Minimum of 2 years of related clinical experience
Proficiency in computer systems, software, and web-based applications
Strong verbal and written communication skills for clear, professional interactions
Excellent organizational and time management abilities
Effective conflict resolution skills and sound judgment in decision-making
Knowledge of care management policies, evidence-based criteria, and levels of care
Nice to Have Skills:
Certification in Case Management (CCM) or Healthcare Management (CPHM)
Experience with patient discharge planning and post-discharge care coordination
Ability to adapt communication and care strategies to culturally diverse populations
Preferred Education and Experience:
Associate’s or Bachelor’s degree in Nursing (preferred)
Previous experience in case management or care coordination roles highly desirable
Other Requirements:
Ability to work a 12-hour shift, 24/7 availability
Physical ability to perform medium work (exerting 20-50 pounds of force occasionally)
Compliance with all federal, state, and local regulations, including OSHA and HIPAA standards
Willingness to work in environments with potential exposure to communicable diseases and hazardous materials
This position starts on March 25, 2026, with an hourly pay rate of $57.71.
