Position Title: Family Medicine Physician – Sonoma County Clinics (Full-Time)
Location: Santa Rosa, CA (Multiple Clinics in Sonoma County)
Position Overview:
We are seeking dedicated and experienced Family Medicine Physicians to join our network of clinics in Sonoma County. This full-time role offers an excellent opportunity to provide comprehensive primary care services across four distinct facilities, helping to meet the healthcare needs of diverse patient populations.
Key Responsibilities:
Provide comprehensive primary care services including preventive, acute, and chronic care management for patients of all ages.
Perform routine examinations and screenings, and interpret diagnostic tests to inform clinical decisions.
Educate patients and families on health maintenance, disease prevention, and management strategies.
Collaborate with multidisciplinary teams to ensure integrated and coordinated patient care.
Maintain accurate and timely documentation of patient encounters in compliance with healthcare standards and regulations.
Participate in quality improvement initiatives and professional development activities to enhance clinical practice.
Qualifications:
Medical degree (MD or DO) from an accredited institution.
Board Certification or Board Eligibility in Family Medicine.
Valid and unrestricted California medical license.
Proven experience in outpatient primary care.
Strong communication and interpersonal skills.
Ability to work collaboratively within a team-oriented environment.
Commitment to providing patient-centered, culturally competent care.
Work Schedule:
Full-time position, approximately 0.8 to 1.0 FTE, working across multiple clinics.
