What You’ll Do
Provide physical therapy services in a home health setting to diverse patient populations
Develop and implement personalized care plans based on patient needs
Educate patients and families on rehabilitation exercises and injury prevention
Document patient progress and maintain accurate treatment records
Collaborate with multidisciplinary teams to ensure comprehensive care
Adhere to all regulatory and safety standards in home health
What You Need Required:
Current Texas state license as a Physical Therapist
BLS certification upon hire
Excellent communication and patient care skills
Preferred:
At least 1 year of experience in home health care
