Location: International Falls, MN
Start Date: Flexible / ASAP
End Date/Duration: Permanent
Shift: 8-hour days, M-F; other options including 4-10's possible
Hours/Week: 38–40 hours
Employment Type: Permanent, Full-Time
License/Certifications: MN Physical Therapist license required; BLS within 1 year of hire
Experience: Minimum 1 year in a health-related field, preferably with EMR/Epic experience
Pay & Benefits
Salary: $80,000 to $115,000 per year, based on experience
Sign-on Bonus: Up to $10,000 for a two-year commitment
Pay package discussed upon application

What You’ll Do
Provide outpatient and inpatient physical therapy services
Assist patients with mobility, strength, and functional improvements
Document patient progress using Epic EMR system
Collaborate with a multidisciplinary team to develop personalized treatment plans
Rotate weekend shifts approximately every 6-7 weeks
Participate in occasional home health or SNF visits as needed
Contribute to a facility with a strong focus on community health and patient engagement
What You Need Required:
MN Physical Therapist license
BLS certification (or obtain within 1 year)
At least 1 year of relevant healthcare experience
Basic knowledge of electronic medical records (preferably Epic)
Ability to work 8-hour weekdays and rotate weekends
Preferred:
Experience in outpatient, inpatient, or SNF settings
Familiarity with Epic software
