Join Our Team as a Nurse Practitioner - Palliative Care
Are you a compassionate and dedicated Nurse Practitioner with a passion for improving quality of life through palliative care? We are seeking a highly skilled Nurse Practitioner to provide onsite hospice and palliative care services at our Medford location, supporting patients with complex chronic and end-of-life needs. This vital role offers the opportunity to make a meaningful impact in a supportive environment while working with a collaborative team committed to excellence in patient care.
In this role, you will deliver comprehensive symptom management, conduct face-to-face admissions and recertifications, and partner closely with Registered Nurses to coordinate patient needs. You’ll serve an average of up to 15 patients per month, overseeing medication prescribing and ensuring compassionate care. The position also involves providing telehealth support for Washington patients during call hours, with backup coverage for Portland via telehealth as needed. The typical schedule runs Monday through Friday, 8 am to 5 pm, with an unpaid lunch break, and includes participation in weekday symptom management call hours, as well as potential weekend call coverage (approximately one weekend per month).
Required Skills:
Valid and current WA and OR Nurse Practitioner licenses
Active WA DEA and OR DEA licenses
Proven experience in palliative, hospice, or related care
Strong assessment, clinical judgment, and medication management skills
Excellent communication and collaboration abilities
Ability to manage a diverse patient caseload efficiently
Nice to Have Skills:
Experience with telehealth modalities
Knowledge of palliative care protocols and symptom management
Previous experience in hospice or end-of-life care settings
Bilingual abilities are a plus
Preferred Education and Experience:
Master's degree or higher in Nursing (MSN or DNP)
Minimum of 2 years of clinical experience in palliative or hospice care
Other Requirements:
Ability to obtain and maintain WA and OR licensure and DEA registrations
Willingness to participate in periodic weekend and evening on-call duties
Willingness to provide telehealth services to Washington patients
