Rate Caps for Submission:
Malpractice
– Must be included in rates.
Orientation
– half the hourly/callback rate kicks in after 8 hours. “Per hour of
orientation or training beyond 8 hours”
Hourly – 321-350 - “Per hour not OT”
Holiday – Must be 1.5x the regular rates “Per holiday hour no OT”

Clinic Mon-Friday 9a-5p Facility Location: 3336 S 4155 W Ste 301 West Valley UT 84120Setting: Outpatient Clinic Trauma Level IIIFTE: 2.0Required Procedures/Job Duties: General OB/GYN procedures amp job duties. Candidates will need to see patients in clinic.EMR: Epic
Support Staff: MA and Sonographer in the clinicReason for Coverage: Departure of existing physician and another physician ramping down in preparation for retirement.
EMR Required Training: Module learn lessons – Must be completed prior to virtual EMR training to attend the training.EMR Virtual training – Only able to attend if Modules are completed before training. Provider is required to schedule and complete their virtual EMR training prior to their first day to avoid missing their first shift.
Travel Preferences see attachment for specific travel guidelines:
· Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available hotel/lodging rates will be reimbursed based on actual expense up to 1.5 times the current GSA rate for the market. https://www.gsa.gov/travel/plan-book/per-diem-rates
· Air Travel: 750/round trip cap
· Rental Cars: 65/day all in cap including taxes and fees regardless of car class.
· Mileage: IRS Standard Rate

Travel exception requests will be denied if this information is not part of the provider presentation.
