Overview:
We are seeking compassionate and skilled LPN/LVN to join our dedicated long-term care team in Kane, PA. This role involves supervising and providing direct nursing care to residents, ensuring compliance with all regulatory standards, and promoting the well-being of our residents. Key Responsibilities:
Collaborate under the supervision of the RN Supervisor to implement and oversee residents' individualized care plans.
Administer and document medications, treatments, and monitoring such as glucose checks, following physician orders accurately.
Transcribe physician's orders promptly and accurately into the electronic health record (EHR/EMR).
Assist with resident rounds, examinations, and treatments as needed.
Collect laboratory specimens (urine, stool, cultures) and prepare them for delivery to external laboratories, ensuring complete documentation.
Review and update care plans regularly in collaboration with nursing assistants, reflecting current resident conditions with clear, measurable goals.
Report significant changes in residents’ conditions immediately to the RN Supervisor and document all observations thoroughly.
Communicate effectively with residents’ families and internal departments regarding transfers, condition updates, and care plans.
Support residents’ participation in scheduled activities to promote holistic well-being.
Assist with post-mortem procedures, including notifying relevant parties and managing resident belongings sensitively.
Supervise nursing assistants to ensure the proper execution of care plans, compliance with infection control practices, and documentation accuracy.
Participate in staff evaluations, contribute to team conferences, and aid in the orientation of new staff members.
Maintain a clean, safe, and organized work environment, including managing treatment supplies, inventory, and reporting maintenance issues.
Support quality assurance efforts by collecting data and monitoring staff performance regularly.
Perform direct resident care as needed, adhering to all safety and clinical protocols.
Qualifications:
Active, valid LPN/LVN license in Pennsylvania.
BLS certification required.
Excellent communication and interpersonal skills.
Demonstrated ability to work compassionately with elderly and chronically ill residents.
Ability to work independently and as part of a team.
Physical ability to perform job duties, including lifting, standing, and mobility requirements.
Knowledge of EMR/EHR systems and medication administration procedures.
Positive attitude and commitment to resident-centered care.
