Required Skills:
Valid Nurse Practitioner license with certification in palliative care.
Proven experience in hospice, palliative, or related healthcare settings.
Pediatric experience (PEDS) is required.
Ability to conduct face-to-face evaluations, including Medicare recertification assessments.
Strong communication skills for effective collaboration with medical and care teams.
Capable of providing home visits and rounding in inpatient facilities.
Competence in patient education and clinical support.
Nice to Have Skills:
Experience working with pediatric patients in hospice or palliative care.
Knowledge of Medicare policies and recertification procedures.
Prior experience in home-based hospice services.
Bilingual abilities or additional language skills.
Preferred Education and Experience:
Master’s Degree in Nursing (MSN) or higher.
Minimum of 2 years’ experience in hospice, palliative, or pediatric care.
Current state licensure with appropriate certifications.
Other Requirements:
Ability to travel locally for home visits, SNF, and care facility rounds.
Flexibility to work part-time, 1-2 days per week.
Willingness to communicate effectively with care managers and interdisciplinary teams.
Compliance with all applicable healthcare regulations and standards.
