Key Responsibilities:
Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders according to regulatory and clinical standards.
Develop individualized care plans utilizing standardized assessments and evidence-based practices.
Monitor and document patient progress, adjusting treatments as needed while ensuring compliance with all documentation requirements.
Evaluate treatment effectiveness and modify services accordingly, referring patients for additional services when appropriate.
Provide consultation and counseling to patients, families, caregivers, and healthcare team members regarding speech and swallowing disorders.
Communicate regularly with supervisors and interdisciplinary teams about patient progress, barriers, and treatment strategies.
Prepare comprehensive discharge summaries and maintain accurate, timely documentation in electronic health records.
Participate actively in patient care conferences, review meetings, staff trainings, and professional development activities.
Supervise and support CFYs or student Speech-Language Pathologists to foster professional growth.
Adhere strictly to ASHA and state licensure requirements, practicing ethically and professionally throughout all responsibilities.
Required Skills:
Valid state licensure as a Speech-Language Pathologist and active ASHA certification.
Proven experience in evaluating and treating communication, cognitive, and swallowing disorders.
Strong clinical assessment skills and familiarity with evidence-based practices.
Excellent communication skills, both written and verbal.
Ability to work independently and as part of a multidisciplinary team.
Proficiency in electronic health records documentation.
Nice to Have Skills:
Previous experience in long-term care or skilled nursing facilities.
Supervision experience with CFYs or graduate students.
Knowledge of assistive devices and adaptive communication tools.
Preferred Education and Experience:
Master’s degree in Speech-Language Pathology or equivalent.
Minimum of 1 year of relevant clinical experience.
Other Requirements:
Willingness to work on a day-shift schedule, starting January 26, 2026.
Ability to obtain necessary supplies, DME, or travel reimbursements as required.
Commitment to ongoing professional development and compliance with all federal, state, and local regulations.
