Position Overview:
We are seeking a dedicated and skilled Physical Therapist to join our team in Colebrook, NH. The successful candidate will evaluate and treat patients with a wide range of conditions, including orthopedic, neurological, complex medical, and other diagnoses impacting strength, mobility, endurance, and overall function. Key Responsibilities:
Conduct comprehensive patient evaluations to assess physical status and establish treatment goals.
Develop and implement individualized treatment plans utilizing therapeutic exercises, modalities, and activities aimed at maximizing patient function.
Monitor patient progress and modify treatment plans as necessary to ensure optimal outcomes.
Educate patients and their families about manageability of conditions and techniques to facilitate recovery and independence.
Collaborate with multidisciplinary teams to coordinate patient care and improve treatment strategies.
Adhere to all safety, documentation, and compliance standards, including maintaining current BLS certification.
Qualifications:
Valid and current Physical Therapy license for the state of New Hampshire.
BLS (AHA) certification required.
Proven experience in orthopedic, neurological, or complex medical rehabilitation preferred.
Strong interpersonal and communication skills, with a patient-centered approach.
Ability to work independently and as part of a collaborative healthcare team.
