  • Description: Summary: Looking for a full time bread and butter Urologist for clinic and call for 3-6 months. Clinic Manager will provide clinic tour/orientation before start date. Applicants should be interested in a wide variety of general cases clinic rounding and OR. Candidates should be collegial team players with good patient and customer service skills. Robot onsite at hospital for cases if needed. Will be sole provider in a fast paced/busy dept. Provider may take hospital call during clinic hours. Assignment Duration: 3-6 months Start: July 28 open dates: Site is looking for one full time provider that can cover the dates below and dates starting week of Dec 15th for 3-6 months. week of: When submitting candidates please list the weeks they are available and how many weeks starting Dec 15th they can be available for. 7/28-8/18/25-299/22-2610/13-1710/20-2510/27-3111/3-711/17-2111/24-28 Total # FTE’s needed: 1 Rate Caps for Submission:Malpractice – Must be included in rates. Orientation – half the hourly/callback rate kicks in after 8 hours. “Per hour of orientation or training beyond 8 hours” Hourly – 450 and below - “Per hour not OT” Holiday – Must be 1.5x the regular rates “Per holiday hour no OT”Nightly Call – 900 and below - “Per night call to hold the pager to include 2 hours gratis” Holiday Nightly Call - 1350 and below - Per night call to hold the pager to include 2 hours gratis” On Call Daily – 4200 and below - “Per 24 hours to include 4 hours gratis 24 Hour Holiday Call Rate - 6300 and below - “Per 24 hours to include 4 hours gratis Call Back – 450 and below - “Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.” Holiday Call Back – 675 and below – “Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.” Minimum length of commitment required: 3 months Minimum number of shifts preferred/month: 5-6 days of call plus clinic Specific/ugent dates needing coverage: asap Est. days per month needing covered: clinic access - as many days as possible 24 call - 5-6 days per month Multiple part time locums accepted to fill schedule: yes Shift Details start/end times please specify if you are looking for Mon-Fri etc.: Clinic: 8a-4p Call: 24 hours 7a-7a 1 in 6 Night call: 5p-7a offsite Facility Location: SMMC Setting: clinic/call Duties: general urology care in clinic and on call Cystoscopy vasectomies prostate biopsy TURP uroflow/cuff bladder scans Ureteroscopy nephrectomy Cystectomy ESWL Botox injections etc. Required Procedures: bread and butter urology robot available Patients per day/volume: 20-25 pts per day Number of surgeries: 20-30/month Robotics? yes Call Volume consults vs callback: 20 consults 80 callback- When on call 20 of patients are new consults and 80 are established patients returning to the ED. Admissions: Hospitalist team present Rounding: Locum will round EMR: Epic Support Staff: Reason for Coverage: open position with difficulty in recruiting current locum found a perm job. Dealbreakers: Providers who are not agreeable to terms in the job desc. Providers who cannot keep up in a fast paced busy enviornment providers who are not agreeable to rates or who have not reviewed the job description and understand pt. volumes and setting. ** incomplete presentations will not be accepted. Travel Preferences: Preferred Lodging: Preferred lodging Air Travel: Local preferred Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where the provider will be traveling from in the travel section of the provider submission. Credentialing Information: Timeframe: 3 months Requirements Board Certifications etc.: board eligible considered Certifications: Credentialing with other facilities? locums automatically credentialed at all facilities not including St. Michaels. Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? no Credentialing Deadline amp Committee Meeting Schedule The credentialing deadline is three weeks prior to the provider’s start date. All required documents must be completed and in hand by this deadline. Providers who will be on vacation out of town or facing any other obstacles that may delay the credentialing process will not be considered. Please ensure these deadlines are taken into account when discussing start dates—only accurate start dates will be accepted. If your provider has any concerns on their background e.g. malpractice criminal history etc. please be aware of the following committee meeting dates when setting their start date:Pierce Regional Credentials Committee Meeting SJMC SAH SCH – Every 3rd Monday MonthlyKing Regional Credentials Committee Meeting SANH SFH – Every 3rd Tuesday MonthlyOne Board Executive Committee Meeting for ALL VMFH – Every 4th Tuesday Monthly Ensuring compliance with these deadlines will help facilitate a smooth credentialing process.
